Assertive Community Treatment Team (ACTT)

This program is an evidence-based model that provides assertive and intensive care to engage individuals with serious and persistent mental illness into personalized, recovery-focused treatment, support, and rehabilitation. The team includes a nurse, prescriber, therapists, recovery support specialists, and more. Care is provided in the community through a highly individualized approach that provides adults with the tools they need to obtain and maintain safe housing, employment, relationships, and relief from symptoms and medication side effects.

Acquired Brain Injury Program

This program enables people with an acquired brain injury to live outside of institutions by providing community-based, non-medical services. Individuals must be eligible for services through either the Connecticut Department of Social Services, ABI Medicaid Waiver or the Department of Mental Health and Addiction Services ABI.

Community Support Program (CSP)

CSP is a community-based rehab model designed to assist adults with serious mental illness to live independently. Staff provide assistance with learning skills to live independently, accessing community resources, finding appropriate and affordable housing, receipt entitlements, linking with appropriate treatment sources, coordination of care with collateral providers, assistance with money management and any other goals they have for their recovery.

Psychosocial Rehabilitation Centers/The Hubs

With locations in Manchester and Enfield, The Hubs are places of hope and healing where adults who receive services at CHR can interact socially in a recovery-oriented environment. Vocational assessment, job placement and coaching, peer support, and social and recreational activities are offered to empower individuals to develop skills that lead to gainful employment in the larger community. Individuals are referred to this program through other programs within the CHR system.

Supported Employment Services

CHR’s Supported Employment Services program assists individuals in exploring and realizing their educational and employment goals. Services include comprehensive vocational assessments, individual and group counseling, and coordination with the business community to broaden each individual’s opportunities. Court Support Services

Family Violence Education Program: We offer court-ordered treatment for those with domestic violence offenses.

Jail Diversion: This program directs adults with psychiatric disabilities and non-violent criminal offenses away from the corrections system and into more appropriate community-based treatment settings. For more information, make one simple call to our Triage Center at 1.877.884.3571.

Housing Services

In an effort to help people find safe and affordable housing that supports their recovery, CHR has developed a robust Housing Services program that includes:

  • Homeless Diversion Services
  • Homeless Outreach and Engagement Services
  • Rapid Re-Housing Services
  • Permanent Supportive Housing Services
  • Promise House: apartment building for young adults who have experienced homelessness

Veterans Resource Service

CHR provides services for veterans, members of the armed forces, and family members who are experiencing mental health and substance use challenges. To learn more, visit our Veteran Services page.

Young Adult Services (YAS)

Similar to ACTT, this multidisciplinary team focuses on case management and outreach services for young adults (18-25) to support their integration into the community. Assistance includes help with establishing a living situation, clinical services and education and/or vocational planning. Individuals are usually referred by the Department of Mental Health and Addiction Services and are often youth who have been in residential treatment through the Department of Children and Families, in foster care and/or in the juvenile justice system.

Warmline

The “Reach” Warmline is a non-crisis, peer-support telephone service. Callers discuss concerns such as boredom, loneliness, fear and anxiety. Callers are directed to CHR’s Triage Center should the need exceed peer communication.  The “Reach” Warmline operates from 6 p.m. to 10 p.m. seven days a week, 365 days a year.
