FROM SPRINGFIELD
- Interstate 91 South
- Exit 47E (Enfield, Route 190 East towards Hazardville/Somers)
- Merge onto Route 190 East/Hazard Ave and continue for 1.9 miles
- At the traffic light turn left into the Hazard Executive Place Plaza (You can easily see that Dunkin' Donuts is on the right hand side of the plaza and Lulu's Pizzeria & Restaurant is on the left of the plaza)
- Community Health Resources is in a new stand alone building in the very back of the plaza (near the trees)
- Visitor parking is available in front of the building
- The general reception desk is on the second floor of the building
- Take the elevator to the second floor
- Partial Hospital Program Visitors may use the separate entrance located at left front of the building. The Partial Hospital program is on the first floor.
FROM HARTFORD
- Interstate 91 North
- Exit 47E (Route 190 East towards Hazardville/Somers)
