Coordinators assist families in developing individual, family-focused service plans and provide coordination, follow-up and advocacy for the children and families they serve. Crucial to the success of this program are the Local Systems of Care with whom CHR works. The Systems of Care are made up of community members, families and providers. Parenting Support Services
By providing services to children and families we assist with healthy and safe parenting. Emphasis is placed on teaching the parent(s) how to care for their child(ren) in a nurturing, safe and caring manner. The Nurturing Families Network provides evaluation to first-time parents delivering at Johnson Memorial Hospital Birthing Center, as well as, developmental screens, telephone support, and home-based parenting education to prenatal and post-natal first time parents in the Greater Enfield area. These support groups are offered throughout the year and focus on parent education and peer support. The Young Parents Program offers weekly group sessions for parents under the age of 25.
