Affordable and safe housing is a critical component to support recovery and optimal health

CHR’s Housing programs help hundreds of individuals and families find and maintain housing, including those who are homeless, about to become homeless, chronically homeless, and those being discharged from state hospitals, jails, and nursing homes, as well as others living in the community.

All of our homeless and housing services use a person-centered approach that tailor services based on individual need and preference. Additionally, all of CHR’s mental health and wellness services are available to individuals in our Housing programs, when needed.

Please note:

  • Referrals for CHR housing programs (RRH and PSH) come directly from the Coordinated Access Network. Referrals for Homeless Diversion and Outreach can be initiated through 211, self-referral, or the community.
  • CHR’s homeless and housing services aim to provide not only short-term basic needs but also support and planning to ensure long-term stability and meaningful connections within their community. Placement in services depends on eligibility, prioritization, and availability.
