family community support services

At CHR, we believe in the saying ‘it takes a village to raise a child’ because we’ve seen how children and families flourish when they have the support of a community behind them. So we’ve made it our mission to build more connections between families and their communities with our family community support services Connecticut.
Currently, we run our Community Support for Families program in Norwich, Willimantic, and Middletown Connecticut. We work with families to help them identify both their strengths and needs so we can connect them to the supports and services they need. We connect families to a range of services that exist in their communities, such as prevention to intervention services. Our staff includes counselors and parent navigators who work hard to develop unique family-oriented care plans to fit any family situation through care coordinating and case management.
Families are the experts in knowing what they need. Our social workers will work with families to find the right kind of help.
Here’s How We help families:
- Strengthen the bond between parents and children.
- Learn about parenting strategies, communication tools and resources
- Know when to seek help and where to find help.
- Develop a strong connection to the community.
- Find resources to help meet basic needs of families.
Referrals for the program come exclusively from DCF’s Family Assessment Response Program.
