Making the call is a first step for individuals or families living with behavioral, emotional and/or substance abuse problems. The CHR Assessment Center offers a single point of contact to readily understand the best treatment and support options available.
Qualified professionals answer the phone and–together with the caller–determine available options and next steps. Our staff assists in scheduling initial appointments, providing directions, answering questions and following up to ensure a smooth transition into services. Making the Call
In a crisis, CHR provides support 24 hours a day, 365 days a year. The Assessment Center offers phone support and mobile emergency teams who travel directly to the person in need to provide intervention, stabilization and access to hospitalization when necessary. For a child in crisis, dial
In a non-emergency situation, for information about CHR services or to schedule an initial appointment, the Assessment Center team is available: Monday, Tuesday and Wednesday
CHR accepts referrals from parents, guardians, family members and friends. We take referrals from schools, hospitals, private providers, primary care physicians, pediatricians, community agencies and emergency service personnel, to name a few. We also welcome

CHR accepts Medicaid, HUSKY Part A & Part B, Medicare and most major health insurances as partial or full payment for services. A sliding scale fee, based on an individual’s or family’s ability to pay for services, is available.
