Enhanced Care Coordination is designed to coordinate and complete a comprehensive clinical evaluation of children in residential care, to facilitate community-based discharge plans, as well as divert unnecessary admissions to shelters, inpatient and/or residential care. The program will also assist in the management of the local service system in order to enhance the community's capacity to provide all necessary community-based services.
This service is offered through Community Health Resources. Children are referred to this program through the Department of Children and Families.
