Care Coordination

Care coordination, part of the statewide Kidcare initiative, provides intensive coordination of care for children, youth and their families with emotional and behavioral disorders who have complex service needs. Care coordinators assist the family in developing individual, family focused service plans and provide linkage, coordination, follow-up and advocacy for the children and families they serve.

 

Crucial to the success of this program and the Mobile Crisis program are the Local Systems of Care with whom CHR works. The Systems of Care are made up of community members, families and providers. Their role is to monitor services, identify service gaps and seek to develop resources to fill those service gaps. Presently there are three Systems of Care, one representing the northern towns of the service area (Bloomfield, East Granby, East Windsor, Enfield, Somers, Stafford, Suffield, Windsor and Windsor Locks) one representing the Ellington, Manchester, South Windsor, Vernon area, and the third representing Andover, East Hartford, Glastonbury, Hebron and Marlborough).
