The System of Care is made up of families, providers, community members and a diverse population of professionals who monitor services, identify service gaps and seek to develop resources to support and advocate for children and families.
A Community Collaborative drives the mission of the System of Care. The collaborative is comprised of voluntary members who work towards attaining and maintaining the core values of the larger system and community.
Three collaborative regions receive administrative support from Community Health Resources. The three collaboratives serve 21 towns collectively. However, each region meets individually and functions independently.
North Central: Bloomfield, East Granby, East Windsor, Enfield, Granby, Somers, Stafford, Suffield, Windsor and Windsor Locks
East of the River: Bolton, Ellington, Manchester, Tolland, South Windsor, Vernon
Route 2: East Hartford, Glastonbury, Andover, Marlborough, Hebron
