Case Manager-IICAPS

Description

Provides assertive and proactive community support services to high need, at risk clients. Provides assistance with daily living activities, such as benefits & money management, household maintenance, general errands & transportation, to assigned clients in need of intensive community based mental health intervention. Functions as team member & provides assistance with activities of daily living, teaches life skills & provides guidance & support to entire team caseload, as needed. Works as liaison between CTT, community based services & family, and assists in linking client with other community services.

Minimum Requirements

Bachelor's Degree Required. Knowledge of principles & practices of community treatment and delivering assertive community outreach, supported housing & non-office based community mental health services to individuals with serious and persistent mental illness, personality disorder or dual diagnosis. Two years counselor experience in mental health setting or equivalent. Ability to function independently in range of settings on an outreach basis. Basic math skills, with ability to budget, maintain financial records & complete complicated benefit related applications. Valid drivers license.
