 Case Manager- Specialized Client Services
Part-Time
Description
This is a part time 20 hour per week position including evenings and weekends.
Provide intensive in-home clinical and case management services for youth and their families.
Completes all required documentation and statistical reports related to client service in accordance with established agency procedure.
Additional duties and responsibilities as required.

Minimum Requirements
BA and 2+ years experience in a mental health setting required.
General knowledge of the principles and practices involved in delivering case management and counseling services that develops independence for individuals living in the community who have mental health needs.
Proven ability to function independently in a range of settings on an outreach basis. Demonstrates competency in age related or specialty issues and developmental needs for each population served.
